hela Sverige Visa alla jobb hos Independent Living Solutions Limited i Stockholm We are looking for an experienced independent health professional to work as a Case Manager with a young lady with an acquired brain injury in central Stockholm.
Case Managers: Coordinate the care, nursing and rehabilitation packages to develop an injured individual's abilities to the fullest. They act as advocates for their clients, providing immediate needs assessments and up to date reports, detailed costings and risk assessments. They assist in the recruitment of therapists and other required professionals to work with the client e.g. architects, speech and language therapists.

Experience: Working with individuals with Acquired Brain Injury (desirable), conducting risk assessments, designing and introducing care plans, goal and objective setting for clients; recruitment and management of care staff and therapists (desirable); working within the community.
Skills and Qualifications: Current registration confirming fitness to practice; relevant qualifications e.g. Degree in Occupational Therapy; up to date CPD; Experienced in the use of IT (email, internet, Microsoft office packages such as Word, Excel); fluent English - spoken and written; excellent communicator.
